The HFA Foundation to Award $5,000 Grants at The HFA Show


One of the noteworthy additions to The HFA Show this year is the awarding of $5,000 grants to two HFA member clubs during the HFA Hall of Fame induction ceremony, which is a fundraiser for the HFA Foundation, the charitable arm of HFA. Representatives from Manitoba Cardiac Institute (Reh‑Fit Centre) in Winnipeg, Manitoba, and Columbia Association in Columbia, Maryland, will receive the grants in recognition and support of their community programs.

The ceremony is at 6:30 p.m. on March 15 at the Omni Hotel. To attend, you must purchase tickets to the HFA Hall of Fame, the proceeds of which will help to fund next year's HFA Foundation grants.


Reh‑Fit Centre: Her Heart Her Way

Reh‑Fit Centre will receive a grant for Her Heart Her Way, a 12‑month behavior‑change program designed for women at risk of heart disease. The program combines in‑person and virtual coaching to help participants build sustainable habits around exercise, stress management, and nutrition.

Participants are referred to the program by their primary care providers. Each participant is paired with a lifestyle behavioral mentor trained in health behavior change, motivational interviewing, and counseling. During the year-long program, mentors help participants set personalized goals, overcome barriers, and connect with additional community resources as needed.

Each Her Heart Her Way participant is matched with a mentor to help her achieve her goals.

The following are the core features of the program:

• Monitoring of physiological, lifestyle, and mental health measures at baseline, six months, and 12 months.

• Ongoing communication with the referring primary care provider.

• Oversight of mentors to ensure consistent, high‑quality support.

• Engagement opportunities, including 12–16 hours of virtual coaching and free access to Reh‑Fit’s group physical activity programs.

• Feedback through a Heart Health Report Card summarizing each participant’s progress for both the participant and her primary care provider.

• Independent evaluation by a university researcher, supported by Reh‑Fit’s data analyst.

As of December 7, 2025, 158 women had completed initial assessments, 104 had completed mid‑point assessments, and 79 had finished the full year. Based on the first 54 women to complete the program, the median Framingham Risk Score dropped from 11.7% at baseline to 8.6%, which is a 27% relative decrease in 10‑year risk of heart attack or stroke. Twenty-nine women moved from the intermediate‑risk category to low risk, and one moved from high risk to intermediate.

These improvements can influence treatment decisions, allowing some women to move from medication and lifestyle intervention to lifestyle intervention alone.

Reh‑Fit plans to publish its findings to advance understanding of women’s heart health and support broader adoption of similar models.


Columbia Association: CA Points Program

The second grant will go to Columbia Association (CA) for its CA Points Program, an initiative that connects student achievement with family access to health, fitness, and recreation opportunities. Through partnerships with local public schools, students who meet academic or engagement benchmarks earn a full CA membership for their entire household, removing financial barriers while reinforcing positive educational and health outcomes.

Each year, Columbia Association hosts an outdoor party for the children in the CA Points Program that includes games and activities.

Since its launch in 2014, 8,176 students and families have earned memberships. Now active in 21 schools, CA Points offers year‑round access to fitness centers, aquatics, ice skating, group exercise, and recreational programming. The program has become a sustainable, scalable model for improving community health through collaboration, accountability, and equitable access.

CA Points includes a range of school‑connected activities and services that include everything from on‑site Hour of CA Play fitness experiences to ice rink field trips, roller‑skating events, swim lesson raffles, and fitness demonstrations at school and community events. Schools work closely with CA to verify eligibility, track participation, and host activities including an annual CA Points Party. The results include a 100% membership activation rate and an estimated 80% use rate.

The program strengthens family‑level health behaviors, increases youth physical activity, and fosters long‑term engagement with community wellness resources. Families gain access to safe, welcoming environments that encourage movement and social connection, while students build physical literacy and confidence across a variety of activities.

CA invests heavily in the program, contributing nearly $1 million annually in membership value, staffing, facility use, and programming. Grant funding will help expand enrichment opportunities such as swim lessons, school skate parties, tennis programs, and arts‑based classes—enhancing the program’s reach and impact for families who need it most.
